Clients requiring a wig via the NHS should read this:

Are you going through chemopherapy or alopecia
If so
Have you seen the chemo nurse or a doctor
regarding a referal.

Yes

No

Have you then been refered by the
surgical appliance department. If
so have you recieved a letter or
a phone call to attend Hair
To Ware.

We are unfortunatly unable to
supply a wig via the NHS without
a referral from the surgical
appliance department.

Have you been informed about the
government prescription charge.

(This is not an NHS prescription so over 65
years of age you may still be charged.)

Ask about exemption details.

Cheque only for prescription
charges made payable to the trust
that has refered you and handed
to your Hair To Ware consultant
on day of appointment or to the
surgical appliance department
prior to your appointment.

Not sure of any of the above

Then ask either one of our Hair To Ware consultants
or your surgical appliance officer

Before making an appointment.
